What to bring:

• Health insurance and identification of various recipients
• A 'mother‐and‐baby notebook' (for medical and welfare records)
• Medicine notebook (If you do not have one, please bring all the medication your child currently takes)

■ We will give medical care only immunization and baby health checkup.

★ We will give medical care allergic patients (bronchial asthma, atopic dermatitis, food allergy, allergic rhinitis)

If you have fever or acute disease, please make an appointment before visiting us.
